The reapplication form must be completed by all applicants who have previously been removed or have resigned from a UK two-year Foundation Programme. Applicants who did not start in post do not need to use the reapplication form.
Reapplication to the Foundation Programme
Applicants must email the completed and signed form to the UKFPO team at helpdesk@foundationprogramme.nhs.uk by the eligibility application deadline (the completed form does not need to be uploaded onto Oriel). Please refer to the eligibility timeline.
