Reapplication to the Foundation Programme

The reapplication form [this downloads a Word document, 57KB] must be completed by all applicants who have previously been removed or have resigned from a UK two-year Foundation Programme. The form template for the 2025 application process must be used. Reapplication forms from previous recruitment rounds will not be accepted.

Applicants who did not start in post do not need to use the reapplication form.

The reapplication form must be signed by the Foundation School Director at the foundation school to which you were previously allocated and trained.

The completed form must be emailed to: helpdesk@foundationprogramme.nhs.uk.

  • If you are an applicant who will be nominated again by your UK medical school, you must submit your reapplication form via email to the UKFPO during the main FP application window 25 September 2024 – 09 October 2025 (12:00 midday BST).
  • If you are an applicant who is applying via the Eligibility application route, you must submit your reapplication form via email to the UKFPO during the Eligibility application window 03 – 25 July 2024 (12:00 midday BST).
