The information you will be asked to provide includes:

Your personal contact details (name, address, email, telephone numbers)
Date of birth
Nationality

Some of this information will be pre-populated from your Oriel profile. Please check that it is correct before moving to the next question.

If the names on your eligibility application form, your ID document, and your primary medical qualification evidence do not match exactly, you must upload evidence of name change/discrepancy.

Refer to the What You Will Need pages for what is required.

You can amend your personal details and contact details at any point through your Oriel dashboard. This includes after you have submitted your application.

1) In the right hand corner of the screen select ‘My Profile’.
2) In the profile options menu, select ‘Contact details’ from the left-hand navigation menu.
3) In the right hand corner, select the ‘edit profile information’ button.
4) Oriel will ask you to re-enter your password.
5) Edit the information you want to change, then click ‘Save’ in the right-hand corner of the screen.
