Non-urgent advice: A five-step check for a focused, useful end of placement conversation
Aim: confirm the foundation doctor has appropriate evidence, identify gaps and agree what should carry forward.
1. Plan feedback in good time
- Recommend a TAB during the first placement, ideally in October, so feedback is available by December
- Consider a PSG feedback cycle in either the first or second placement, led by the CS
- A satisfactory outcome for each is required by ARCP; planning early allows time to repeat either if needed
2. Check the evidence
- Around 5 to 10 SLEs, with enough evidence to link to the FPCs
- Reflections and e-learning
- Core learning hours
- Simulation experience where possible, recorded on the LEARN form if appropriate.
3. Check the breadth of learning
- Mental health and social wellbeing, as well as physical health
- Chronic disease and community experience, as well as acute medicine
- Patient safety considered in their learning
- Any children and young people experience described and expanded
4. Review quality improvement
- Where possible, check that quality improvement work considers sustainability
- This may include environmental sustainability or the sustainability of healthcare
5. Close the loop
- Support FD to record key points in the summary narrative
- Where evidence or experience is absent, agree a clear PDP for the next placement.
