Clinical and educational supervisors’ start of placement meetings
These are formative events, intended to make sure you receive all the information and support you need when you start your foundation training and all new placements. The meeting forms will be completed in your portfolio by your supervisors, following regular meetings with you and regular reviews of your e-portfolio.

Clinical and educational supervisors’ reports
These form the basis of your summative assessments (that is, whether you are progressing as expected or you need extra support) and will be completed in your portfolio by your supervisors, following regular meetings with you and regular reviews of your e-portfolio.
