Form to confirm your primary medical qualification (PMQ)
You must use this form template if:
Your primary medical qualification (PMQ) certificate has not yet been issued to you, for example, if you are still a medical student or have very recently qualified from medical school.
Or:
Your PMQ certificate does not display the date you qualified from medical school.
You must upload and attach a completed copy of the form to your eligibility application in Oriel.

Re-application to foundation training form
You must complete a re-application form if you have previously been removed or have resigned from a UK two-year Foundation Programme. You must upload and attach a completed copy of the form to your eligibility application in Oriel.
You don’t need to complete one if you did not start in post.
