In this section of the form you are asked to state whether you have any unspent and spent convictions, investigations and/or warnings regarding your fitness to practise.
If you answer Yes you will also need to later share the details of this with your foundation school and employer if you are appointed to a programme. Please use the Fitness to practise form which can be downloaded from the Oriel resource bank and email the completed form to your foundation school and employer after you have accepted an offer.
Non-urgent advice: Fixed penalty notices – road traffic offences
The GMC has provided guidance that states that you do not need to declare any road traffic offences where you have accepted the option of paying a fixed penalty notice.
All other fixed penalty notices must be declared.
