You are required to read the declaration statements and tick the box for each to confirm you have understood the declarations.
Once a decision has been made concerning your appointment, the recruiting organisation will not retain this declaration for any longer than is necessary. This declaration will be kept securely and in confidence. Access to this information will be restricted to designated persons within the NHS employing organisation who are authorised to view it as a necessary part of their work.
