You must provide evidence of your primary medical qualification (PMQ).
You must provide either:
Non-urgent advice: Ensure you get it right
No other forms of evidence will be accepted other than those specifically listed.
Your evidence must clearly demonstrate the details of the degree(s) you were or will be awarded by your medical school. Do not upload transcripts, statements of results, copies of licentiate certificates, or evidence of entry onto a medical register (in any country) as these are not required and will not be accepted as evidence of your PMQ.
If you do not provide satisfactory evidence of your PMQ, your eligibility application will be deemed ineligible.
PMQ certificate
You must upload a scanned copy of your PMQ certificate if it has been issued to you. The document you upload must:
- Clearly demonstrate the details of the degree(s) you were awarded by your medical school
- Be clear and easy to read all text with no blur, glare, or shadows
- Be straight and properly aligned – not tilted or skewed
- Show all edges and corners
- Be free of obstructions
Full colour copies are preferred; however, you may provide a certified or notarised copy (for example, it has been certified/notarised by a solicitor, notary public, or other appropriate authorised person or institution). Apostilles are not required for the purpose of your UK Foundation Programme application.
If your names do not match across your eligibility application form and all supplementary documentation
If your name has legally changed: You must upload evidence of an legal name change (marriage certificate, divorce decree, deed poll certificate etc.). You must upload and attach this evidence to your application and enter any previous name(s) into the personal details section of the application form. Please also refer to the guidance below for any document(s) that are not in English.
If your name has not legally changed (for example, you were known to your medical school by another name): You must upload a letter from your medical school to explain the reason for the discrepancy and to confirm that you are the same person. The letter must appear on the letterhead of the medical school, be signed and/or stamped, and dated by the medical school. You must upload and attach this letter to your application form.
Middle names or family names such as A/P or D/O are not part of the ID check. Refer to the Guide to names and naming practices for further guidance.
Using a digital PMQ certificate
The UKFPO will accept a digitally-issued copy of your PMQ certificate if it clearly demonstrates the details of the degree(s) you were awarded by your medical school.
Document(s) that are not in English
If you have document(s) that are not in English, you must provide an English translation from an official organisation for each document. The original language document(s) and translation(s) must be combined into one document. The UKFPO advises you to follow the GMC’s guidance on translating documents that aren’t in English.
If you do not provide satisfactory translation(s), your eligibility application will be deemed ineligible.
Form to confirm the details of your PMQ
If your PMQ certificate has not yet been issued to you (for example, you are still a medical student or you have very recently qualified from medical school), you must upload a completed copy of the form to confirm the details of your PMQ. The document you upload must:
- Be the UKFPO 2026 Form to confirm the details of your primary medical qualification (PMQ)
- Be fully completed – all fields marked with an asterisk (*) are mandatory
- Be signed and stamped by the medical school Dean or an authorised official in the Registrar’s office

Previous applicants
If you have submitted an eligibility application in a previous recruitment year, please note that you cannot re-use a previous PMQ confirmation form or Dean’s statement. Only the version of the form for 2026 applications will be accepted.

If your PMQ appears on the GMC’s list of overseas medical qualifications they may accept
If your PMQ appears on the GMC’s list of overseas medical qualifications they may accept, you must submit evidence that your PMQ meets the GMC’s criteria for an acceptable overseas qualification. This evidence must be an email sent to you by the GMC, with the sender’s email signature(s) and email address(es) clearly displayed. This evidence will be assessed on a case-by-case basis by the UKFPO.
The deadline to provide this evidence is 12:00 (midday, BST) on 04 September 2025. If this evidence is required but not provided by this deadline, your eligibility application will be withdrawn from the process.
You are strongly encouraged to contact the GMC as soon as possible in order to gain this evidence.
If your PMQ appears on the GMC’s list of overseas medical qualifications they do not accept
The UKFPO can only accept applications from applicants whose PMQ is accepted by the GMC. A list of the primary medical qualifications that are not currently accepted by the GMC can be found on the GMC’s website.
If your PMQ appears on both lists
Some qualifications appear on both the GMC’s list of medical qualifications they may accept, and the list of medical qualifications they do not accept.
