The information you will be asked to provide includes:

Your personal contact details (name, address, email, telephone numbers)

Date of birth

Nationality

Some of this information will be pre-populated from your Oriel profile. Please check that it is correct before moving to the next question.

If the names on your eligibility application form, your ID document, and your primary medical qualification evidence do not match exactly, please then you must upload evidence of name change/discrepancy.

You must refer to the eligibility guidance on how to evidence a name change/discrepancy.

You can amend your personal details and contact details at any point, including after submission of your application through your dashboard on Oriel.
1) In the right hand corner of the screen select ‘My Profile’.
2) You will be presented with the profile options menu, select ‘Contact details’ from the left-hand navigation menu.
3) In the right hand corner of the screen, select the ‘edit profile information’ button.
4) You will be prompted to re-enter your password.
5) You can then edit the information that you wish to amend, finally click on the ‘Save’ button which is on the right hand corner of the screen.
